TrueNTH Implementation Toolkit

This toolkit was developed by the University of Southampton as part of the TrueNTH UK Supported Self-Management and Follow-Up Care Project. The guide shares a wealth of resources to support the implementation of stratified follow up in prostate cancer care. Many of the resources can also be tailored for use in other cancer services.

University Hospital Southampton NHSFT Patient Triggered Follow Up (PTFU) Resources

UHSFT has nearly a decade of experience in delivering stratified follow up (known locally as PTFU) in cancer care. UHSFT have developed a number of resources that can be utilised to support the effective delivery of of follow up.

My Medical Record (MyMR) Resources

My Medical Record is an award winning IT platform developed by UHSFT. It can be used for the ongoing monitoring of many cancers and other chronic conditions. It also has a website that allows patients to log into their own record, see results, complete assessments and contact their nursing team.
